BEFORE: Speaking of oral hygiene, perhaps we should start with bacteria that enter with food and settle on the teeth and gums. Isn't it an exaggeration when we are scared that these bacteria have a destructive effect on the entire body?

YUB: Yes, the bacteria in the oral cavity are quite pathogenic, but this is our normal microflora. As soon as the bacteria get under the gum, they affect the state of immunity in general. Microparticles of food that have fallen under the gum become a breeding ground for microorganisms - this is how tartar is formed, which causes inflammation in the periodontium, destroys the cement in the root of the tooth, which leads to the development of carious processes and diseases that cause loosening and loss of teeth (gingivitis, periodontitis, parodontosis). Therefore, it is very important to do professional cleaning once a year with the help of ultrasound and the Air-Flow system. This helps prevent further tartar build-up.

BEFORE: What is the right way to brush your teeth?

YB: Sweeping movements from gum to incisal edge. From top to bottom (lower jaw, on the contrary, from bottom to top), from red to white. The same rule applies to the inner surface of the teeth - from the gum to the cutting edge. The chewing surfaces are already cleaned at the end in a circular motion. And you need quite a bit of paste - about a pea, this amount is enough, the main thing is the correct method of brushing your teeth.

TO: Do you need to clean your tongue?

YB: Definitely! It is necessary to clean the back of the tongue, as it accumulates up to 70% of bacterial plaque, so it requires the same hygiene as teeth.

TO: Which brush is better to choose? Usually doctors advise medium hardness.

UB: The hardness of brushes, toothpaste, rinses and additional hygiene products are selected strictly individually for each patient, depending on the condition of the oral cavity.

BEFORE: Which types of whitening do you consider the most effective and gentle?

YUB: One of the latest achievements in aesthetic dentistry is laser whitening, as it is the only method of gentle intervention in the hard tissues of the tooth to achieve a whitening effect. When exposed to laser light, we get the "evaporation" of the pigment from the tissues of the tooth, without violating the integrity of the enamel. After laser whitening, pain and sensitivity are reduced to almost zero. The number of procedures, the time of laser exposure are calculated individually for each patient.

TO: Does it make sense to use whitening pastes at home? Do they harm the enamel?

YUB: It must be understood that whitening pastes for daily use only return the natural whiteness of the teeth, that is, they remove plaque from coffee, tea, nicotine. People with sensitive teeth and damaged enamel should not use these pastes.

TO: If a tooth is lost, is it necessary to grind the neighboring ones? Wouldn't it be better to insert a pin?

AB: A pin is an implant that is implanted into the bone tissue, imitating the root of a natural tooth. If we talk about the prosthetics of one tooth, then there is a rather gentle way - an immediat prosthesis, a removable prosthesis for one tooth. The adjacent teeth are not damaged, aesthetically it looks great, the tooth is held quite firmly, but this type of prosthetics can be called an “ambulance”: a person suddenly lost a tooth, and he has a wedding tomorrow … Then we help in this way. This is a temporary closure of the defect until the final prosthetics on the implants.

BEFORE: So the alternative is a bridge? Or is it an implant? By the way, is deep penetration into bone tissue harmful?

AB: This is not harmful at all. The implant, of course, is preferable, because it is less deep penetration than the natural tooth root, which sometimes reaches 1.5 cm. The implant is usually no more than 8-12 mm.

But before installing it, it is necessary to conduct an MRI and a strict calculation of the areas where the implants should be. Making a "Hollywood" smile is easy. But to calculate everything correctly, to accurately determine the place where each tooth should stand, while not damaging the surrounding tissues, muscles, nerves, without violating the overall structure, is a very long and painstaking work. Therefore, implants are very expensive.

BEFORE: If the doctor does not recommend implants, which method of prosthetics do you consider the best?

AB: It all depends on the individual characteristics of the patient's oral cavity. There has been no revolution in dentistry for a long time - since the invention of cermets. All innovations are connected with the invention of new materials, even better, more natural. They are trying to improve not only in terms of aesthetic features, color depth, but also in terms of fabric strength. To allow natural wear to occur simultaneously with your teeth.

BEFORE: What materials do you consider the most perfect?

AB: The list of new materials can be long. For example, today it is metal-free ceramics.

TO: Is it as strong as cermet?

AB: No, and its aesthetic properties are much better. And the physical ones are pretty good too. After all, their teeth are chipped, even iron ones can break! Therefore, there is no need to talk about durability. I got a bone in a bun with raisins - and that's it, a tooth broke. Of course, both porcelain and other metal-free crowns are afraid of point loading, for example, when you bite off a thread. And with a uniform load, such crowns last a very long time. But subject to regular (once every six months) medical examinations and professional hygiene.

Progressive technique is now considered to be the pressing of ceramics on metal. This is the same cermet, only porcelain is pressed onto a finished metal frame. One of the most recent inventions is zircon. This is an expensive and complex computer technology, but zircon has a very high aesthetic effect.

One of the sparing technologies is endocrowns, these are overlays for the crown part made of composite material. Provided that everything is done correctly, the tooth under the onlay feels great.

TO: Too many people put off going to the dentist, wincing at the memory of the sound of a drill. Is boron finally obsolete?

IL: Of course, we cannot do without boron. But we are ready to convince the patient that dental treatment can be painless thanks to the use of modern and effective anesthetics. But, if the sound of a mechanical bur still causes fear and stress, we use ultrasound and a laser. But this is effective only in the treatment of superficial damage, for example, with very superficial caries or "sealing" of fissures, that is, cracks in the enamel.

TO: Which filling methods are considered the most modern and effective?

IL: The newest fillings currently in use are microhybrid filling materials, they are the most aesthetic, work well, hold color and are strong enough. And the method of their installation is light-curing. The latest diagnostic and treatment equipment that we use is highly technological, without heating the tissues surrounding the treated area, without unpleasant sounds. And it makes the treatment more comfortable.

TO: Is it possible to minimize the loss of "source material", i.e. the tissue of one's own tooth?

IL: Unfortunately, sometimes these losses are inevitable in the process of full treatment. But thanks to working with a special microscope, we manage to preserve he althy tooth tissues as much as possible. In addition, the use of such a microscope allows you to put a seal and carry out the restoration of the tooth more efficiently.

BEFORE: Can I get rid of stains on my teeth?

IL: First you need to determine the nature of these spots. If this problem is of an aesthetic nature, laser whitening can remove color irregularities. In advanced stages, when stains are difficult to remove with whitening, this cosmetic defect can be corrected with veneers. These thinnest porcelain plates that replace the outer layer of teeth help to solve many aesthetic problems. Veneers are one of the most modern methods of aesthetic restoration of teeth.

TO: What could be the reason for the formation of chips, cracks, erosion of the enamel?

IL: The causes of such damage can be divided into chemical and mechanical. Eating foods and drinks that contain high amounts of acids, “aggressive” brushing with powders, abrasive whitening pastes, overloading the teeth due to malocclusion or missing teeth, and, of course, mechanical damage. The main thing in the treatment of these problems is the elimination of the causes. Professional treatment is quite long, with the involvement of specialists such as a hygienist, orthodontist and orthopedist. We can prevent any disease. The main thing is to be on time!
